I am currently accepting new patients (either in person or via telehealth depending on the patient). Call me to set up an appointment, or email me to schedule a time to discuss an appointment. No referrals from primary care physicians needed.
Please have the consent forms filled out before first appointment. Links to these are below, but I occasionally make edits to these documents so it’s best to double check with me before you fill this out. I should also soon have a new feature so you can fill these out online rather than printing them out (save a tree!).
Patients age 65 & up should fill out “Geriatric Consent Forms,” and all others should use the “Adult Consent Forms.”
I am very excited to be utilizing pharmacogenomic testing which is relatively new to psychiatry. This does not need to filled out ahead of time, as I don't always order this test for every patient. Office address is listed below. There is street parking and an attached garage (which you can access via Marion Street just south of Boren Ave). PLEASE BE AWARE THAT EVEN IF THERE IS A SIGN SAYING LOT FULL, THIS DOES NOT APPLY TO PATIENTS. YOU MAY STILL PARK HERE. On a busy day, you may need to circle the garage a bit until someone leaves. I am not able to validate any parking at this time.
Please arrive no more than 5 minutes before your scheduled time. Due to COVID, the office is trying to minimize waiting room traffic. There will be COVID screening including temperature checks for all patients & escorts. You must wear a mask & use hand sanitizer upon arrival, but we do have extra masks if needed.
